Pathways to Community Counseling (PCC)
Pathways to Community Counseling (PCC) services are provided in a youth’s home or in an amenable community location by a licensed behavioral health clinician. The location of PCC services helps destigmatize mental health counseling and therapy. Services are intended to stabilize and support the youth and family to deter more intensive interventions and to support the client and family. PCC is an analytical, focused, goal oriented, and needs-based clinical intervention that addresses emotional and behavioral challenges of clients with moderate to high needs. It is a short-term, solution focused intervention that addresses presenting behaviors resulting in the youth and their family, care giver, and/or guardian gaining insight and improving function at home and in the community.
The PCC services are part of a comprehensive plan of care that is driven by the client and family to address assessed needs. They are rendered at a time of day and location optimum to the family. The interventions are contoured to meet the unique needs of the youth and are reflective of their cultural values and norms and utilize the strengths of the youth and family. Visions and Pathways wants to be part of the closing racial disparity gap. PCC helps destigmatize mental health services, increase services to LGBTQI+ youth, and increase access to mental health services for Latino and Black families.
The primary objective of the interventions is symptom reduction and individual skill development with the goal of restoring or maintaining the youth’s functioning. PCC clinicians provide youth and their families with an array of interventions such as psycho-education, negotiation and conflict resolution skill training, effective coping skills development, healthy limit-setting, emotional regulation skills development, stress management, self-care, symptom management, problem solving skills, and skill building that enhances self-fulfillment.
The Behavioral Health Department provides at-risk youth with:
- a variety of therapeutic approaches to support mental, spiritual and physical health,
- opportunities for personal growth, and
- resources to improve social skills.

Prosperity Living Skills Program
Prosperity is a family-centered home visitation program for youth ages 14 to 21 throughout northern and central New Jersey. Participants receive independent-living skills education, career counseling, and employment preparation through weekly workshops, mentoring groups and one-on-one support.
Many Prosperity participants will soon “age-out” of foster care. Visions and Pathways has developed relationships with corporations who provide volunteers to assist clients in reaching their personal goals. Ultimately, Prosperity helps decrease the probability of long-term dependence on public assistance, incarceration, and other detrimental life experiences.
Through home visits, one-on-one, counseling, mentoring and workshops, youth receive instruction in:
